- Download the Code of Conduct Form (WORD)
- Enable Editing (if necessary)
- Print as many copies as needed for all players, parents, coaches and program directors to read and sign
- Program directors then need to keep a hardcopy of all forms on file, for the duration of the season, to be made available to League officers should they be needed
